I have just installed 2.6.5 having first uninstalled 3.0.0
With a few minor exceptions 3.0.0 played all files with no issues.
Now all my ffmpeg transcodes give...

Media Server Error: An HTTP error (500) has occurred

I'm using ums on whs v1, ps3 renderer and only ffmpeg transcode. All other transcode options are disabled. These are the same settings I have been successfully using since 2.5.2.

Errors are expected if you rollback from 3.x to 2.x without a clean install

Yes, doing a refresh fixed the install and now all media is playing as intended. Please confirm when a refresh install is required as rebuilding the cache can take some time !!
