So the problem was 100% network. The 2.4 GHz frequency for WiFi in my neighbourhood must be satured to high heaven. Also the Powerline adapter was severely crippled by old wiring in the house I think.

Spent the day working on this, narrowed it down to my network. I'm almost certain it's the Powerline adapter now, my building is quite old and it's likely throttling the speed.

Here's the Tracelog. This is about ~5 minutes into the show, took 15 minutes to get there amongst all the lag.
Common settings:
Maximum Buffer: 400 [Same result on 200]
Disable Subtitles
MEncoder as default [Tried Remux videos with Tsumixer both on and off, same result. probably worse with it off ...

So I tried both Plex and Serviio with the same results. It's gotta be something with my computer, I totally don't get it. I even tried two separate Wi-Fi sticks in my desktop to no avail, it couldn't even stream/transcode 720p files which are typically fine on Wireless N.

Just tried the half duplex, no go unfortunately :(
Also updated my NIC again to the latest driver, same result. This is what my status screen looks like, max buffer is set to 400 mb at the moment but it doesn't seem to make any difference.

I've tried the new Alpha version and unfortunately I'm running into the same scenario. I have the mpeg2 selection set to automatic for wired and even tried increasing the heap size. I've tried 200, 400 and 600 buffer but unfortunately none of them seemed to work ...
