We are in the process of releasing 4.0.3 so it should be out within a day and hopefully it fixes this issue for you. If not, I've added a bunch of extra logging stuff to 4.0.3 to make it easier to solve these things in the future, so if it's still an issue with the upcoming 4.0.3 release please upload fresh logs
