Trying to use the Avisynth/Mencoder engine. If I use Mencoder the subs work fine.

With AviSynth engines, subtitles are controlled by your computer codecs/settings

Mencoder is also not playing the default audio/subtitles.

If you upload logs for that I can have a look

We go by language preferences instead of defaults though so that might explain it. By default, UMS will show subtitles for videos unless the audio language is English. You can customize the language preferences on the Subtitles settings subtab on the Transcoding Settings tab

Here's the debug file when I use just Mencoder. I currently have the audio language preferences blank. I would prefer it if it would play MKV with the defaults I've already set since some things I watch in English and others I don't.

UMS is behaving properly here since most users don't create their own files and the "default" flags in many files users have are unpredictable. You can go into the TRANSCODE folder at the top of any folder and choose whichever language combination you want, and customize your preferences in UMS to fit your most common scenario.

For example if you want to usually prefer Japanese audio over English then you can set the audio priority to "jpn, eng, und"

Could you point me in the right direction to properly configuring Avisynth/Mencoder?

If you install K-Lite Codec Pack it will give you a link to "DirectVobSub" in the K-Lite Configuration folder in the Start Menu. There you should be able to configure your subtitle preferences, and you can also do it in realtime since when AviSynth is running you should get a DirectVobSub icon in the taskbar.
